Requirements for Power of Attorney Attestation

 

1. Duly filled in typed Power of Attorney mentioning CNIC/POC numbers and residential addresses of Executant and Attorney with two witnesses (Attach CNIC/POC and passport copies of Attorney as well), signatures of 2 witnesses with their CNIC copies. If POA consists of more than one page, each page must be signed by the Executant.

2. 2x recent photographs (3 X 4 size light blue background).

3. The applicant/s has/have to come in person to the Embassy and sign or put a thumb impression on the POA and the register of the POA.

4. Original valid Pakistani Passport, CNIC, Permit of Stay (visa) in Indonesia along with photocopies.

5. Complete address, CNIC Numbers, passport numbers, and contact numbers of the Executant and Attorney both. A data form can be taken from Embassy and filled in and submitted along with POA
